Acting as a patient advocate, the full-time remote Care Coordinator will manage patient inquiries, coordinate admissions, and facilitate communication among interdisciplinary teams to ensure a seamless continuum of care. Key responsibilities Support patient experience preadmission and throughout the continuum of care by screening patients and relaying program recommendations Ensure timely follow-up with patients and families, maintaining open lines of communication Facilitate internal transfers between sites by managing referrals and communicating pertinent information to treatment teams Required qualifications High School diploma required; Bachelor's degree preferred Two years of customer service experience, preferably in a healthcare setting Proficiency with computers and software platforms, including EMRs Knowledge of medical/psychiatric/chemical dependency diagnosis and treatment processes Familiarity with managed care and Joint Commission standards preferred
